Here’s all you do to apply by mail: If you are a Vermont resident turning 65 and you have Medicare Parts A and B, you can choose between two Vermont Blue 65 Medicare supplement plans. Print, complete, sign and return the application (format is Adobe PDF). Please remember to include a photocopy of your Medicare card and a check in the amount of your first month’s premium with your application in order to establish coverage.* *If you are transferring from a Blue Cross and Blue Shield of Vermont policy or a policy from The Vermont Health Plan (TVHP), you do not need to include a check with your application. Vermont Blue 65 coverage does not apply to spouse or dependents. |
